For added convenience, Dr. Adelina Duka and Dr. Arvi Duka are now happy to offer all the necessary new patient paperwork here online. All you have to do is download them, print them out at home, and have them filled in before you arrive. After turning them in to our front desk team, you’ll find that your wait time is significantly shortened.
